Medical Insurance Information and Declaration of Health
Admission to KCDC’s Summer Intensive program is contingent upon the participant's sound physical and mental health.
The Summer Intensive program provides a basic medical insurance policy for all participants. This policy covers only illnesses or medical conditions that arise during the participant’s stay in Israel. The provided medical insurance does not cover treatments necessitated by the recurrence of any chronic condition, nor does it cover any illness or condition that existed prior to the participant's arrival in Israel. In the event of illness or injury, participants may consult a doctor at the medical clinic located in Kibbutz Ga’aton or at a clinic in Nahariya. All prescriptions issued under these circumstances will be covered by the insurance. Important: It is the participant’s responsibility to collect prescribed medications from a pharmacy in the nearby city of Nahariya.
Exclusions:
The insurer will not cover the following treatments or services:
Physiotherapy, mechanotherapy, hydrotherapy, alternative therapies, homeopathy, acupuncture, or chiropractic treatment. Optometry, cosmetic or corrective surgery, experimental procedures. Periodic check-ups, medical treatment programs, or alternative medicine. Gum therapy and/or surgery, dental treatment (except emergency dental care). Mental health treatment, including psychiatric disorders
All other treatments or services not covered by the policy will be the participant’s financial responsibility. If a participant requires therapy, ongoing treatment, or continuation of prescribed medications during the program, they must provide a detailed medical letter from their attending physician. Since medications may not be available in Israel under the same trade name as in the participant’s country of origin, the full pharmacological name of all required medications must be provided. The cost of such medications will remain the participant's responsibility.
Insurance Activation
To be covered under the insurance, participants must complete and submit the required details via the Harel Insurance Company link one month prior to the start date of the Summer Intensive program.
Declaration of Health
In addition to completing the insurance form, participants must marks the statement that is relevant to them:
I hereby declare the following:
- I am in good health and fully capable of participating in a program that requires up to six (6) hours of intensive dance activity per day, five or six days a week.
- I acknowledge that the insurance provided covers only medical expenses within the State of Israel. If I wish to obtain additional coverage for personal accidents, disability, life insurance, or travel to my country of origin, I understand that I must arrange and pay for such insurance independently and at my own risk.
- I would like to declare and update the course staff that I have an illness, medical condition, allergy, or any health-related limitation. Therefore, I will send an email to the summer course with a detailed description of the issue and include an updated confirmation from my attending physician. My doctor will confirm that I have no medical or physical disabilities, do not require ongoing medical treatments or other types of care, and that I am medically, mentally, and physically fit to participate in an intensive dance program.
